Please fill up the form online on date 13-Jan-2020 itself and submit a printout of the form in school office along with the form fees on 15-Jan-2020.

Timing:- 12:30 pm to 03:00 pm.

It is mandatory for both the parents along with the candidate to meet the HEADMISTRESS on 18 JANUARY 2020 as per the schedule which will be given to you at the time of submission of the printout in the school office.

Girl Child will be given preference at the ratio: 6:4
REQUIREMENTS AT THE TIME OF SUBMISSION OF FORM.

• APPLICATION FEE: - Rs.700/-

• A duly attested copy of Birth Certificate from the Municipal Corporation/Nagar Nigam.

• Baptismal Certificate for a Catholic/Christian Child and a letter from the Parish Priest.

• Scheduled Caste/Tribe Certificate for SC/ST child.

• Proof of address (Adhar Card/Driving License/Passport/Ration Card)

• Two passport size photographs of the child.

• A proper recent photograph of the entire family with the date printed (Parents and all the children) taken during the last six months only.
(Age limit: 3.5 years to 4.5 years as on 1st April 2020).
